Select stages:
Play through the game until finishing a Subway A1 level.
Go back to the main menu and enter Up, Down, Left, Right, 
Up, Left, Down, Circle.

Invincibility:
Pause the gameplay and enter L1, L1, L2, L2, R1, R1, R2, R2.
Note: After an auto-save, type the code again to re-enable.

Get all weapons and full ammunition:
Pause the gameplay and enter L1, L2, R1, R2, Triangle, Circle, X, Square. 

Unlimited ammo:

Neverending bullet time:

Repeat the code to disable.

Last Challenge bonus level:
