After you kill all the Supply Captains Dong Zhuo will rebel.
Finish the level to unlock him.

4th weapons:
Get weapons on stages with at least 3 stars under the Hard difficulty level
or any stage on the Chaos difficulty level.

Last Gaiden stages:
As the indicated faction, complete the following task in Stage 7 to unlock
their Last Gaiden stage.

Sengoku: Get 1,000 KOs.
Wu: Rescue the Sun family in less than three minutes.
Wei: Defeat the defected Da Ji in less than one minute.
Shu: Escape from the burning castle in less than two minutes
(all friendly officers as well).
